SCHOOL NURSE & MEDICAL FORMS
Our School Nurse is here daily from 9:00 - 3:00. If your child is going to absent or tardy, you are asked to either call the absence line ~ 419-382-3803 or her extension via the school phone number ~ 419-381-1775 ext. 134. You may also email email@example.com prior to 9:00am. If she does not hear from you by this time, you will receive a call to verify your student's whereabouts.
Appointments ~ If your child has an appointment during school hours, please send a note to school stating the time you intend to pick up your child. Your child will meet you in the Nurse's Office where you (or the person designated in your note) will sign out the student. If your student has an appointment and will be late to school, please call the absence line or email attendance to avoid unnecessary calls. For students leaving for an appointment and returning to school afterwards, students should report to the Nurse's Office to be taken off the absence list and to get an admit slip back to class.
Tardiness ~ All students entering the building after 8:30am must report to the Nurse's Office. By doing so, the student will be taken off the absence list.
Medication ~ All medication must have written consent of the parents and physican. The exception being generic ibuprofen or acetaminophen (Tylenol). This can be administered at school if you indicate this on the Blue Family Emergency Procedure Card.
Emergency Information ~ Each family is required to complete a Blue Emergency Card each year, along with a white emergency medical consent form for each child. Emergency information (blue card and consent form) are sent home the first week of school or the first day your student is enrolled at St. Patrick of Heatherdowns (if later than the start of school).
New Students ~ If your child is new to St. Patrick of Heatherdowns school, please be sure that a complete immunization record has been forward from your previous school.
Pre-Kindergarten and Kindergarten students are required to have a current physical form with all shots up to date for their age.
